The Illinois School Code requires all students entering Kindergarten, 6th, and 9th grades to have an updated physical examination and complete immunizations against preventable communicable diseases. Dental exams are also required for students entering 2nd and 6th grades.

Please mail, fax, or drop off your children's health records to the office before the first day of school.

State law mandates that children who do not have complete and current health records are not to be permitted to attend school. Immunizations may be obtained from your family physician or:
