The Illinois School Code requires all Kindergarten, 2nd, and 6th grade students to submit a dental examination form by May 15th of the current school year.

If your child has been recently examined by a dentist, please have the dental office complete and fax an Illinois Dental Examination Form to the school nurse.

If your child has not yet had an exam, please schedule one with your family dentist and return the completed form to school after the appointment, or you may enroll your student in the upcoming school dental clinic.
