Physical Examination
Up to Date Immunizations- including 3 doses of Hepatitis B Vaccine

Kindergarten
Physical Examination

Up to Date Immunizations- including 2 doses of Varicella Vaccine

Second Grade
Dental Examination

Grades Six through Twelve
1 dose of Tdap Vaccine
3 doses of HepB Vaccine

Sixth Grade
Physical Examination
Dental Examination
Up to date immunizations- including 2 doses of Varicella Vaccine, 1 dose of Tdap, 1 dose of Meningococcal Vaccine

Ninth Grade
Physical Examination
Up to date immunizations- including 2 doses of Varicella Vaccine

Dental Examination

Twelfth Grade
2 doses of Meningococcal Vaccine, unless the first dose was given after 16 years of age, in which case only one dose of Meningococcal Vaccine is required.
