Welcome new students and families!

Parents should be prepared to present the following documents and forms when enrolling their child(ren) in the Byron School District for the first time.

_____ Proof of Residency – One required of these:

Closing Papers
Mortgage Statement
Current Apartment Lease – must be dated during the school year of registration. Lease must be signed by all parties and include landlord’s name and phone number.

_____ Proof of Residency – TWO current bills from this category:

Phone bill
Cable TV bill
Water bill
Electric bill
Gas bill

*Items NOT accepted: driver’s license, checkbooks, rent receipts, insurance papers,credit card statements

_____ Birth Certificate - certified original copy (hospital copy not accepted).

_____ Physical examination - required for all new out-of-state, new preschool, kindergarten, sixth, and ninth grade students. The physical form must be signed by an Illinois physician. (Form available on Nurses' Page)

_____ Dental examination - required for all new out-of-state students and for students entering kindergarten, second, and sixth grades. (Form available on Nurses' Page)

_____ Eye examination - required for all new out-of-state and kindergarten students. Eye exams must be completed by a licensed optometrist or ophthalmologist. (Form available on Nurses' Page)

_____ Proof of immunizations against communicable diseases as required by the Illinois Department of Health – required within one year for all new students, new preschool, kindergarten, sixth, and ninth grade students. Students entering twelfth grade must have record of meningococcal conjugate vaccination (MCV).

_____ Custody Papers (if applicable)

_____ IEP or Special Education Records (if applicable)
